CCRH (the “Company”) has created this Job Application Privacy Policy in order to demonstrate its firm commitment to your privacy. The following describes our information gathering and dissemination practices that apply to information you submit through the job application.

Personal Information & How We Use Personal Information:
The process of applying for a position through our job application may result in our collecting some or all of the following personal information:
  • first and last name;
  • mailing address;
  • telephone number;
  • birth month;
  • birth day;
  • some or all digits of your Social Security Number;
  • tribal affiliation;
  • employment history;
  • education;
  • transcripts;
  • motor vehicle records;
  • desired salary;
  • employment preferences; and

  • This information is shared only with relevant CCRH employees and with no other party, unless required by law. In addition, this information will not be publicly displayed. The personal information may be used to identify candidates for positions at CCRH, assessing your capabilities and qualifications for a job, conducting reference checks, conducting background checks, and contacting you about career opportunities. The CCRH may provide the information to federal, state, local or other government agencies that have jurisdiction over matters related to employment. The CCRH will share the information needed to complete all necessary background screening with third party vendors contracted for such screenings. The CCRH does not provide any other information it collects on applicants unless it is required to do so by a valid court order, governmental agency order or authorized by you in writing. If the CCRH employs you, it will retain this information and may use it and disclose it to others for personnel, administrative or other purposes related to your employment with the CCRH. All of the information you provide to the CCRH, including personal information is collected and saved in a database and/or employment file according to the CCRH’s recordkeeping retention policy.

    Information Submitted:
    You are responsible for the content of the information you provide to us and you must ensure it is legal, honest, truthful, accurate and not misleading in any way.

    Data Security:
    The CCRH takes reasonable efforts to protect the security of the data contained in our system with physical, electronic, and managerial procedures.

    Revising and Removal of Information:
    If you think the information the CCRH has about you as a result of your submission through this job application is either incorrect or out-of-date, please resubmit the updated information by updating your application.
    All information will be stored according to the CCRH’s recordkeeping retention policy and any information removed according to such policy will be disposed of in such way to ensure no unauthorized release of said information.

    Changes to this Job Application Privacy Policy :
    We reserve the right, at any time and without notice, to add to, change, update or modify this Job Application Privacy Policy, simply by posting such change, update or modification on the application. Any such change, update or modification will be effective immediately upon posting to the application. We encourage you to periodically review this Job Application Privacy Policy in order to keep apprised of our current privacy practices.
